At Berry Health, we believe every patient deserves to be treated with dignity, respect, and transparency. This Patient Bill of Rights outlines the standards of care and service you can expect from us.

Your Rights as a Berry Health Patient

1. Right to Quality Care

You have the right to receive quality healthcare services from licensed, qualified medical professionals. All healthcare providers affiliated with Berry Health are independently licensed in the states where they practice.

2. Right to Informed Consent

You have the right to be fully informed about your diagnosis, treatment options, potential risks and benefits, and alternative treatments before consenting to any care. You have the right to refuse treatment at any time.

3. Right to Privacy and Confidentiality

You have the right to have your personal health information kept confidential in accordance with HIPAA and applicable state laws. Your medical records and communications will be protected and shared only as outlined in our Privacy Policy and Notice of Privacy Practices.

4. Right to Access Your Records

You have the right to access, inspect, and obtain copies of your medical records maintained by Berry Health and its affiliated providers.

5. Right to Respectful Treatment

You have the right to be treated with courtesy, respect, and dignity, regardless of your race, ethnicity, national origin, religion, gender, sexual orientation, age, disability, or ability to pay.

6. Right to Transparent Pricing

You have the right to clear, upfront information about the costs of your care, including subscription fees, medication costs, and any additional charges, before services are rendered. Berry Health does not have hidden fees.

7. Right to Continuity of Care

You have the right to continuity of care, including a dedicated physician who reviews your progress and adjusts your treatment plan over time.

8. Right to Communication

You have the right to communicate with your healthcare provider about your health concerns. You have the right to receive timely responses to your questions and to be informed about changes in your care.

9. Right to Safety

You have the right to receive care that is safe and effective. All compounded medications prescribed through Berry Health undergo third-party analytical testing and are produced in FDA-regulated facilities.

10. Right to File a Complaint

You have the right to voice concerns or complaints about your care without fear of retaliation. Complaints may be directed to hello@berryhealth.com.

Your Responsibilities

As a Berry Health patient, you are responsible for:

  • Providing accurate and complete health information to your provider.
  • Following the treatment plan recommended by your physician.
  • Notifying your provider of any changes in your health condition.
  • Keeping your account information current and your login credentials secure.
  • Treating Berry Health staff and providers with courtesy and respect.
  • Meeting your financial obligations in a timely manner.
