Telehealth Informed Consent
This Telehealth Informed Consent (“Consent”) applies to the telehealth services provided through the Berry Health platform by independent, licensed healthcare providers affiliated with Berry Health.
What Is Telehealth?
Telehealth involves the use of electronic communications, information technology, and other means to enable healthcare professionals to evaluate, diagnose, and treat patients in remote locations. This may include the use of secure video conferencing, phone calls, messaging, and the exchange of medical information electronically.
Benefits of Telehealth
Telehealth offers several potential benefits, including:
- Improved access to healthcare by allowing you to remain at home or in another comfortable location.
- More efficient evaluation and management of your health concerns.
- Convenience and reduced travel time and expense.
- Timely access to specialists and medical professionals licensed in your state.
Risks of Telehealth
As with any medical procedure, telehealth carries potential risks, including but not limited to:
- Information transmitted may not be sufficient to allow for appropriate medical decision-making.
- Delays in evaluation and treatment could occur due to technology failures.
- Security protocols could fail, causing a breach of privacy of personal medical information.

- You have the right to withhold or withdraw consent to telehealth services at any time without affecting your right to future care or treatment.
- The laws that protect the confidentiality of your medical information also apply to telehealth.
- You have the right to inspect all information obtained and recorded during a telehealth interaction.
- You may request that the healthcare provider provide care in person rather than through telehealth, understanding that this may require referral to another provider.
Your Responsibilities
By consenting to telehealth services, you agree to:
- Provide accurate and complete information about your medical history, current conditions, medications, allergies, and other relevant health information.
- Inform your healthcare provider of any changes in your condition.
- Follow your healthcare provider’s instructions and treatment plan.
- Contact emergency services (911) if you experience a medical emergency.
Compounded Medications
Compounded medications prescribed through Berry Health are prepared based on specific patient needs or in response to drug shortages. These medications are produced in FDA-regulated facilities; however, they have not been individually approved by the FDA for safety, efficacy, or quality.
All formulations undergo third-party analytical testing to confirm potency and strength as labeled. You should discuss the specific risks and benefits of any compounded medication with your healthcare provider.
Consent
By using Berry Health’s services, you acknowledge that you have read and understand this Telehealth Informed Consent. You voluntarily consent to receive telehealth services through the Berry Health platform and understand the potential risks, benefits, and limitations described above.
You understand that Berry Health is a technology platform and does not directly provide medical services. All Healthcare Services are provided by independent, licensed medical professionals.
